Vale Osteopathy is committed to protecting your personal information (any information that is capable of identifying you) and this privacy policy relates to our use of your personal information collected either from you in person, via 3rd party referrals or information you give us by letter, e-mail, SMS or over the telephone. We have a legal obligation to collect and process this data when it is adequate, relevant and limited to what is necessary.

Who we are
Vale Osteopathy diagnose and treat health conditions. Treatments are carried out in accordance with the Institute of Osteopathy’s patient charter http://www.iosteopathy.org/osteopathy/the-patient-charter/. The practice may also provide other treatments, about which our staff will be pleased to provide more details.
The information we collect:
At the point of enquiry or booking we may ask you for;

your telephone number (home and or work), either landline or mobile or both

your e-mail address these will all be stored securely on the clinic database.
At your appointment at the clinic, we will ask for information regarding your general health, your previous health and information regarding the condition for which you are seeking advice.
We will also ask for information regarding any activities you undertake, your employment and any medication you take.
We will also record the findings of a physical examination. These will be on typed notes stored securely.
How we use this information:

To contact you in regard to you’re on going treatment. To contact you if new information or treatments become available that may be of benefit to you.

We may pass information with your permission to other medical professionals who may be involved in your care; this may include GPs, consultants or other Health and Care Professionals. This information may be passed on to you by email, it will be password protected and we will take all reasonable precautions to transmit the information securely. If you are supplied with a letter, the protection of this information is your responsibility.

We take all reasonable steps to ensure that our information is kept up to date and rectified if necessary.

How long do we keep personal information?
We have a legal obligation to retain records for 8 years after the conclusion of treatment.
If the record relates to a child or young person, the records must be kept until the patient’s 25th birthday. We may retain records indefinitely for use if you return for another episode of care within 8 years. As recommended by the General Osteopathic Council.
How do we protect your information?
We take organisational and technical security measures to protect the information against unauthorised disclosure or unlawful processing (our website contains links to external sites, these sites have their own privacy policies and we are not responsible for their content, therefore you should check their privacy policy prior to submitting any personal information).
Your rights
You are entitled to view the personal information we hold about you and to have any discrepancies rectified. You can do this by written request to the address at the start of the policy.

Consent
Through agreeing to this privacy notice you are consenting to Vale Osteopathy processing your personal data for the purposes outlined. You can withdraw consent at any time by using the postal, email address or telephone number provided at the end of this Privacy Notice.

Disclosure
Vale Osteopathy will keep your personal information safe and secure, only staff engaged in providing your treatment will have access to your patient records, although our administration team will have access to your contact details so that they can make appointments and manage your account. Vale Osteopathy will not disclose your Personal Information unless compelled to, in order to meet legal obligations, regulations or valid governmental requests. The practice may also enforce its Terms and Conditions, including investigating potential violations of its Terms and Conditions to detect, prevent or mitigate fraud or security or technical issues; or to protect against imminent harm to the rights, property or safety of its staff.

Data storage
All Data is held in the United Kingdom. Vale Osteopathy does not store personal data outside the EEA.
Patient records are stored on Cliniko, a cloud-based clinic database. Cliniko protect patient data subject rights in accordance with GDPR.

To access what personal data is held, identification will be required
Vale Osteopathy will accept the following forms of identification (ID) when information on your personal data is requested: a copy of your driving licence, passport, birth certificate and a utility bill not older than three months. A minimum of one piece of photographic ID listed above and a supporting document is required. If Osteopaths is dissatisfied with the quality, further information may be sought before personal data can be released. Complaints
In the event that you wish to make a complaint about how your personal data is being processed by Vale Osteopathy you have the right to complain to us. If you do not get a response within 30 days, you can complain to the ICO.
